HMP has established a subcommittee to work collaboratively on the development and implementation of the triennial Community Health Needs Assessment (CHNA). In 2019, HMP is formalizing the Community Health Improvement Planning (CHIP) process.
2019 CHIP Timeline
- Complete and post Request for Information (May 10, 2019)
- Review Proposals (Week of May 13th)
- Interview prospective candidates (Week of May 20th)
- Consultant Selection and Contract Execution (By June 20th)
- CHIP begins July 1, 2019
